Eligibility for Subsidized Assisted Living
A person may be eligible for Assisted Living if they:
- Are eligible to receive VCH home support services; and
- Are able to direct your own care and make decisions about your daily activities and needs; and
- Are able to live independently but require some help with day-to day activities; and
- Do not behave in ways that put the safety of others at risk; and
- Are able to take direction in an emergency and use an emergency response system.
In order to access subsidized Assisted Living, you need to be assessed by a case manager at the Community Health Centre nearest you. Please note: you cannot access subsidized Assisted Living by contacting the assisted living providers directly.
Assisted Living is not an option for people:
- Who are unable to direct their own care or make decisions about their day to day activities (unless they are living with a spouse who can make decisions on their behalf); or,
- Who have care needs that exceed what can be provided in assisted living, or whom need 24 hour professional care (e.g., nursing).
Are there Age Limits or Restrictions?
- One of the goals of the Assisted Living program within VCH is to prevent or delay admission to complex care. Therefore, subsidized assisted living units are primarily geared towards seniors who require this type of supported environment.
- Some VCH sites may be appropriate for younger, physically disabled individuals.
Can I access Assisted Living from the Hospital?
- Assisted Living is a community environment without the extent of care staff support available in hospital. As such, a client needs to return home to the community and demonstrate they can live independently prior to accessing subsidized VCH assisted living units.
- Typically, clients are not able to access VCH Assisted Living units directly from hospital, e.g., acute care, transitional care unit, respite care.
- If you are in hospital and are interested in Assisted Living, ask to speak with a hospital Social Worker who can provide you with contact information for the Community Health Centre nearest you.
